Overview of the New York State 1115 Waiver
The NYS 1115 Waiver presents a significant opportunity for Harvest Home Farmers Market and other community-based organizations across the 14 regions where the State plans to implement the initiative.
Through the Waiver, community-based organizations like Harvest Home Farmers Market will be able to join a Social Care Network (SCN) — a coordinated system designed to provide high-quality, evidence-informed, and accessible services that address social drivers of health. These networks will be managed by a lead entity that ensures the effective delivery of services to Medicaid members, while facilitating data and technology sharing between CBOs, SCNs, New York State, and other key stakeholders to measure the program’s impact on health outcomes.
SCNs will screen Medicaid recipients to identify those eligible for support programs that address health-related social needs. Harvest Home Farmers Market will offer food and nutrition services to those who qualify.
